Dylan was born July 15, 2004 in Port Charlotte, Florida. He was a 7th grade student at Port Charlotte Middle School.
He is survived by his loving family, his mother, Tasha Marie Rose of Port Charlotte; father, Brad Ronald Jack of Bradenton, FL; a brother, Ayden J. Mazzone of Port Charlotte; two half-brothers, Alex and Dallas Jack of Bradenton; a half-sister, Sara Jack of Bradenton; maternal grandmother Brenda Jean Rose of Port Charlotte; maternal grandfather, Duane Edward Horner of Port Charlotte; paternal grandmother, Vicki Jack of Bradenton; paternal grandfather, Robert Jack of Michigan; and many friends.
Funeral services will be held Sunday 2:00 PM, November 26, 2017 at the Port Charlotte United Methodist Church, 21075 Quesada Ave., Port Charlotte, Florida 33952. Interment will follow at Restlawn Memorial Gardens in Port Charlotte. Friends and family are invited back to the church after the interment for a reception and gathering to celebrate and remember Dylan’s life.

Thank you for being so sweet to me.. you probably didn’t know, but I was going through a lot when I lived in port charlotte, and I was currently living in a foster home because of the situation I was in. But it’s not about me, it’s about how kind you were. you’ve always been so nice to me.. you’ve never commented on a flaw I was very insecure about, and I greatly appreciate that. I was bullied for it, and you never joined in. You always were a funny kid, sweet, and kind. It sucks it’s been 6 years since you’ve passed… I would have reached out saying thank you everything you’ve done. No matter how little it was. And most importantly, I still think about you from time to time.. your memory will never dull… but it should forever stay put. If there’s a god, I hope he gives you another chance at life, because you’re a good soul that deserves another chance. Thank you for everything, and thank you to your mom who raised such a sweet, kind, happy boy<3
